Medication Reconciliation Service

I’ve read that patients need to bring a list of their medications with them to the hospital and the clinic yet many don’t, expecting the EMR to have that information. I’m surprised that this is the case. Why don’t EMRs have this information? Is there no database that any hospital can access or request data from about a patient’s current medications? Would the country need a national health service to offer this service?
